Treatment for chocolate cysts

Electrocautery: Electrocautery is used to remove endometrial lesions.

Local treatment: It can relieve dysmenorrhea, infertility, lumps and pelvic nodules caused by chocolate cysts.

Surgical treatment: It is divided into conservative surgery and semi-conservative surgery. Conservative surgery can preserve the fertility of the uterus and remove or destroy the lesions; semi-conservative surgery can control pain and preserve ovarian function.

Laparoscopic treatment: If the traditional combination of Chinese and Western medicine is not effective and the lesion is difficult to remove surgically, laparoscopic surgery can be used for treatment.

Radical surgery: This type of surgery mainly involves a total hysterectomy, which can more thoroughly remove the lesion. Surgical treatment of ovarian chocolate cysts is currently an effective method that can increase the chance of conception.

Under direct monitoring by color Doppler ultrasound, avoid large blood vessels, use a special needle to puncture the cyst, drain the cystic fluid, rinse it, and inject anhydrous alcohol and other hardeners to make the ectopic endometrial cells on the cyst wall coagulate and necrotize, so that bleeding will no longer form chocolate-like cystic fluid during each menstrual cycle, and the cyst wall will adhere and close. If endocrine drugs are used for injection, the principle is to inhibit ovulation, which also inhibits bleeding of endometrial cells. Over time, the cyst wall will adhere and absorb. This cyst wall itself is formed by the peritoneum, without other tissues, and will not cause other harm in the abdominal cavity.

There are two ways to puncture chocolate cysts. One is to insert the needle through the vagina. This method requires exposing the perineum and is inconvenient for the patient, but the image is clear and no anesthesia is required. The other method is to insert the needle through the abdominal wall. During the operation, the needle tube is highly stable and convenient for the doctor to operate.

No matter what needle insertion method is used in ultrasound interventional treatment of chocolate cysts, it will not cause overflow of cyst fluid. In addition, it is rinsed clean. According to relevant reports, it is very safe and effective as long as it is performed carefully by a specially trained ultrasound doctor. After observing hundreds of cases of chocolate cyst puncture treatment, no cases of implantation or recurrence were found.
