Director Bao Zhuocan of the Department of Metabolism at the National Taichung Hospital said that it is not uncommon to find in the outpatient clinic that some people who are on a weight loss plan unexpectedly find themselves suffering from chronic diseases when they go to the hospital for treatment due to physical discomfort after a sudden drop in weight. In addition to deliberate and planned factors, many diseases are also manifested clinically as weight loss. Generally, meaningful weight loss refers to a 10% reduction in the original stable weight within 6 months without deliberate weight loss, or a rapid decrease of 5% within one month.

Weight loss can generally be divided into three reasons, including excessive energy consumption in the body, excessive energy loss in the body, and reduced food intake. There are three main reasons for weight loss due to excessive energy consumption in the body, including hyperthyroidism, pheochromocytoma, and strenuous exercise. Weight loss due to loss of energy intake into the body is mostly due to diabetes or gastrointestinal malabsorption syndrome. There are many clinical causes of malabsorption syndrome. For example, fatty diarrhea caused by chronic pancreatitis, small intestinal lymphoma, pancreatic tumors, radiation therapy damage, bile duct obstruction, etc.

Director Bao Zhuocan said that if a patient loses weight and has an increased food intake, it is usually due to hyperthyroidism, diabetes or malabsorption syndrome. Weight loss due to decreased food intake is the most common cause of weight loss. Insufficient food intake is mostly due to decreased appetite, which is also a common manifestation of many diseases. Possible causes of decreased food intake include malignancy, chronic infection (AIDS, tuberculosis, endocarditis), hypercalcemia (malignancy, hyperparathyroidism, sarcoidosis), uremia, obstructive gastrointestinal disease, anorexia, adrenal insufficiency (primary or secondary), pernicious anemia, dementia, and depression.

In young patients, the most common causes of unexplained weight loss are diabetes, hyperthyroidism, anorexia or chronic infection, while in the elderly, they are malignant tumors, mental disorders (such as dementia, depression), chronic diseases, etc. According to statistics, if elderly people experience unexplained weight loss, about 30% is caused by malignant tumors, about 30% is caused by mental and neurological diseases (such as depression and early dementia), and 30% cannot be found out even after a long time of investigation. Therefore, weight loss in the elderly should never be taken lightly.
