Key points for diagnosis of hyperprolactinemia

Key points for diagnosis of hyperprolactinemia

Breast examination is extremely important for patients with hyperprolactinemia, especially for patients with menstrual disorders, amenorrhea and infertility. The examiner needs to squeeze both breasts by hand to understand the situation of milk leakage. If milk leakage is found on one side, other reasons should be found. During gynecological examination, attention should be paid to whether there is reproductive organ atrophy. So, what are the key points for diagnosing hyperprolactinemia?

Experts say that the diagnosis of hyperprolactinemia mainly includes the following points:

Detailed medical history

The patient should be asked in detail about her past menstrual history, marital status, pregnancy and delivery history, and breastfeeding history.

Physical examination

Breast examination is extremely important, especially for patients with menstrual disorders, amenorrhea and infertility. The examiner needs to squeeze both breasts by hand to understand the situation of milk leakage. If milk leakage is found on one side, other reasons should be found. During gynecological examination, attention should be paid to whether there is reproductive organ atrophy.

Clinical manifestations

1. Menstrual disorders or amenorrhea: generally secondary amenorrhea, some of which occur during childbirth and miscarriage, and there is withdrawal bleeding after using progesterone. 2. Infertility: often the only complaint of patients when they visit the doctor. 3. Galactorrhea: due to the small amount of galactorrhea, most of them are discovered by the doctor's careful examination. A few patients have spontaneous excessive milk. 4. Low estrogen symptoms: some patients may have a series of symptoms similar to menopausal syndrome due to low estrogen levels, such as paroxysmal hot flashes in the head and chest, irritability, loss of libido, dry genitals, and difficulty in sexual intercourse. 5. Manifestations of primary diseases: patients with pituitary tumors may experience headaches, nausea, and visual impairment. Hypothyroidism or renal insufficiency will cause symptoms of the corresponding diseases.

Laboratory and other special tests

1. Serum hormone level measurement: (Blood sampling time 8:10 am) Measurement of PRL level in blood is a screening test method for this disease. 2. Ovarian function test. 3. Image examination. 4. Visual field examination.

The above is an introduction to the key points of diagnosis of hyperprolactinemia. I hope it will be helpful to you.
