1. Uterine dilation and drainage and estrogen therapy

For chronic endometrial tuberculosis and senile endometrial tuberculosis, the cervical dilation method can be used to facilitate the drainage of uterine secretions and eliminate the cause. Elderly patients can also use a small amount of estrogen, such as taking 1 mg of diethylstilbestrol orally every day, and after 7 to 10 days, change to 0.5 mg every day for one month.

2. Intrauterine administration

For married patients with endometrial tuberculosis, intrauterine medication can be used. Before the operation, the size and position of the uterus should be determined by bimanual examination. After disinfection of the vulva and vagina and detection of the depth of the uterine cavity, a sterile catheter should be inserted into the uterine cavity from the cervical os, with a depth less than 0.5 cm. The selected antibiotic should be slowly injected into the uterine cavity through the catheter. After all the drug solution has entered the uterine cavity, the catheter should be removed, and the patient should lie flat or raise his hips for 1 to 2 hours. This method should be used once a day and discontinued during menstruation. Since this method can make the drug act directly on the endometrial tuberculosis site, it often has significant therapeutic effects.

3. Anti-infection treatment of endometrial tuberculosis

First of all, the most effective antibiotic should be selected based on the results of bacterial culture and drug sensitivity test. Before the pathogenic bacteria are identified, gentamicin and metronidazole can be used in combination to effectively treat Escherichia coli, other aerobic bacteria and a variety of anaerobic bacteria.

4. Clear uterine cavity residues and other foreign bodies

Endometrial tuberculosis that occurs after childbirth or abortion, if there is suspected placental tissue residue, should be removed immediately while using antibiotics, but it is better to gently protrude the residue in the uterine cavity, and try not to perform curettage. Curettage can be performed only when the antibiotics reach a certain dose and tuberculosis is under control to prevent the spread of tuberculosis. If there is active bleeding in the uterus, the uterine cavity can be cleaned with the use of a large amount of antibiotics. For those with contraceptive devices in the uterus, they should also be removed as soon as possible to eliminate the lesions of primary endometrial tuberculosis and control the spread of endometrial tuberculosis.
