The lesions are mostly confined to the endometrium, with scattered millet-like nodules in the early stage. In very rare severe cases, the lesions invade the myometrium. The uterus is normal in size or slightly smaller, and there is no abnormality in appearance. Tuberculous nodules can be seen under the endometrium scraped under the microscope, and caseous necrosis may occur in severe cases. A typical tuberculous nodule has 1 to 2 giant cells in the center, which are arranged in a horseshoe shape, surrounded by epithelial cells, and infiltrated by a large number of lymphocytes and plasma cells on the outside. The characteristic of endometrial tuberculous nodules is that the glands around the tuberculous nodules are insensitive to ovarian hormones, showing persistent hyperplasia or insufficient secretion. Severe endometrial tuberculosis may cause caseous necrosis and superficial ulcers, resulting in the destruction of most or all of the endometrium, and scars may form later, and the endometrium loses all its functions, resulting in amenorrhea. Indications for surgery (1) The symptoms of fallopian tube and ovarian abscess subsided after drug treatment, but the lump did not disappear, and the patient felt that the symptoms recurred. (2) Patients whose drug treatment is ineffective and who develop tuberculous abscesses. (3) A large encapsulated effusion has formed. (4) The endometrium is extensively destroyed and anti-tuberculosis drugs are ineffective. (5) For patients with tuberculous peritonitis complicated by ascites, surgical treatment combined with drug therapy is beneficial to the recovery of peritoneal tuberculosis.
