Pathogenesis of menopausal functional uterine bleeding in TCM

Traditional Chinese medicine believes that the cause of menopausal functional uterine bleeding is basically the same as that of adolescent functional uterine bleeding. However, due to the gradual decline of kidney qi, organ dysfunction, and spleen weakness in menopausal women, they often have no power to control and cause metrorrhagia. Functional uterine bleeding is closely related to the kidneys and is related to the liver and spleen. The following are the common pathogenesis of menopausal functional uterine bleeding in clinical practice:

Liver and kidney deficiency

Congenital liver and kidney deficiency or sexual exhaustion that damages the spermatorrhea, early marriage and early childbearing, too many miscarriages, etc., lead to deficiency of liver and kidney yin fluid. Yin deficiency causes internal heat, which burns the Chong and Ren meridians, forcing blood to flow abnormally and causing functional uterine bleeding.

Spleen and Kidney Yang Deficiency

The spleen and kidney are damaged by deficiency of yang in the body or long-term illness, overeating, overwork, etc. The spleen controls blood, and yang deficiency will make it unable to control; kidney yang deficiency will make it lose its function of sealing and storing, resulting in Chong and Ren meridians not being solid, causing functional uterine bleeding.

Blood stasis

Cold stagnation leads to blood stasis, and qi stagnation leads to blood stasis. Stagnant blood blocks the Chong and Ren meridians in the uterus, and new blood cannot return to the meridians, so it does not flow through the normal path and functional uterine bleeding occurs. Blood stasis will turn into heat if it accumulates for a long time, burning the blood vessels and causing continuous bleeding, or accompanied by blood clots.

Tips: TCM syndrome differentiation should be based on the amount, color, quality and accompanying politics of bleeding to distinguish whether it is deficiency, heat or stasis. Deficiency is mostly caused by spleen deficiency, weak kidney qi, weak yang qi, and deficiency of both qi and blood; heat is mostly caused by yin deficiency with heat, or dampness and heat obstruction; stasis is mostly caused by qi stagnation and blood stasis, cold stagnation and blood stasis, or injury-induced stasis.
