Ectopic pregnancy is a common gynecological acute abdomen. Surgical treatment requires attention to the following aspects.

1. Control bleeding

Patients with ectopic pregnancy who have severe internal bleeding or even shock should undergo laparotomy in a timely manner to find the diseased fallopian tube and quickly clamp the mesosalpinx with an oval clamp to control bleeding and speed up the infusion to increase the patient's blood pressure in a short period of time. If the patient has lost a lot of blood, blood volume should be replenished while actively correcting shock and performing surgical rescue.

2. Choose the appropriate procedure

There are two main surgical procedures for ectopic pregnancy: conventional salpingectomy on the affected side, which is mainly suitable for women who do not have fertility requirements; and conservative surgery, which is suitable for women who have fertility requirements and aims to preserve the function of the fallopian tube. It can be divided into two types according to the different implantation sites of the fertilized egg, namely, opening the fallopian tube after removing the embryo, local suture or electrocoagulation hemostasis, and squeezing the fimbria of the fallopian tube to remove the embryo. It should be noted that no matter which surgical procedure is used, careful observation must be made during the operation, and the patient's vital signs and abdominal symptoms should be monitored after the operation. The blood β-chorionic gonadotropin level should be checked 24 hours, 3 days, and 7 days after the operation. If the index decreases, methotrexate and other drugs can be used to prevent the occurrence of persistent ectopic pregnancy.
